Memphis Health Plan / Church Health Center ??????


 

MEMPHIS Plan participants are assigned by our staff to a primary care physician and local hospital. Participants see their assigned primary care physician for regular office visits. The primary care physician may refer the patient to a specialist if needed. Physicians coordinate lab and diagnostic testing as well as hospitalization. Participants go to their assigned hospital for emergencies and surgical care when recommended by their primary care physicians.
To enroll employees on the MEMPHIS Plan, employers must:
  • Make sure employees meet the MEMPHIS Plan’s eligibility requirements.
  • Agree to pay at least $10 of each employee’s $50 monthly fee.
  • Collect each employee’s portion of the monthly fee and remit the total amount to the MEMPHIS Plan.
Self-employed people also may be eligible to enroll in the MEMPHIS Plan.

What Is Covered

  • Primary and specialty care
  • Hospitalization and emergency room services
  • Diagnostic tests, lab work and x-rays provided by designated providers
  • Dental services provided by the Church Health Center dental clinic
  • Prescription discount cards
Participants are also provided access to the Church Health Center’s optometry, counseling and wellness services. However, payment is required from the participant and is generally determined on a sliding scale according to income and family size.

Who Is Eligible

Employers:

To participate in the MEMPHIS Plan, employers must:
  • Be located in Tennessee.
  • Have no more than 200 eligible employees.
  • Employers may not drop current insurance coverage to offer the MEMPHIS Plan or allow employees currently covered by insurance to drop their coverage. Employers may offer the MEMPHIS Plan to employees who are unable to afford the insurance provided through the employer or do not qualify.

Employees:

To participate in the MEMPHIS Plan, employees must:
  • Make no more than 200 percent of the poverty level, which is currently $430 a week for an individual (employees with higher incomes may still qualify depending on family size).
  • Work at least 20 hours a week.
  • Not have TennCare or private insurance coverage for at least six months prior to enrollment.
  • Have worked at least three consecutive months with their current employer.
  • Not have serious pre-existing conditions that require surgery or extensive immediate care.

Self-Employed:

To participate in the MEMPHIS Plan, self-employed workers must:
  • Live in Tennessee.
  • Be currently uninsured (no private or governmental insurance coverage).
  • Make no more than 200 percent of the federal poverty level based on family size and income as shown on their federal tax return.
THE CATCH:

This Plan Is NOT HEALTH INSURANCE!

Although the program is excellent for those in need, doctor's in the network donate services through the program therefore leaving patients in need having to go where the Health Center has assigned. For adults, you will either go to Methodist or St. Francis. For children, the assigned facility is LeBonheur.

If your meet the qualifications, beware that if you go outside of where assigned, then your visit will not be covered under this health plan. Additionally, the plan does not cover out of state expenses and if you have to go in for emergency treatment or specialist care, if the doctor under the plan has not assigned such, then your treatment will not be covered either...
